چکیده

Natalizumab inhibits lymphocyte trafficking across the blood–brain barrier, reducing relapse rates by up to 70% in patients with relapsing-remitting multiple sclerosis (RRMS). Discontinuation of the drug, usually due to pregnancy or risk of progressive multifocal leukoencephalopathy (PML), can trigger the return of disease activity or, occasionally, a fulminant immune reconstitution inflammatory syndrome (IRIS) as lymphocytes re-enter the CNS. We describe a case of life-threatening IRIS following natalizumab cessation in pregnancy, but complete clinical rescue with no adverse sequelae following drug restart in the second trimester.

Natalizumab-Related Progressive Multifocal Leukoencephalopathy-Immune Reconstitution Inflammatory Syndrome: A Case Report Highlighting Clinical and MRI Features.

Multiple sclerosis (MS) patients treated with natalizumab often face the uncommon but severe complication of developing progressive multifocal leukoencephalopathy (PML). PML may be further complicated by immune reconstitution inflammatory syndrome (IRIS) after the removal of the drug. Since both PML and IRIS are associated with high morbidity and mortality rates, early clinical and radiological...

Update on PML and PML-IRIS occurring in multiple sclerosis patients treated with natalizumab.

The use of natalizumab to treat multiple sclerosis (MS) has been associated with the development of progressive multifocal leukoencephalopathy (PML), with 242 PML cases reported as of May 3, 2012. Fortunately, rapid withdrawal of the drug and administration of plasma exchange has allowed survival in many of these patients, but a new problem, immune reconstitution inflammatory syndrome (IRIS), h...

Severe inflammatory disease activity 14 months after cessation of Natalizumab in a patient with Leber’s optic neuropathy and multiple sclerosis – a case report

BACKGROUND Leber's hereditary optic neuropathy (LHON) co-occuring with multiple sclerosis-like disease (LHON-MS) is suggested to be a separate disease entity denoted Harding's disease. Little is known about the response to initiation and discontinuation of potent immunomodulatory treatment in LHON-MS. CASE PRESENTATION We describe a LHON-MS patient with 27 years disease duration who developed...

P9: Cervical Spinal Cord Extraction in Patients with Multiple Sclerosis Using Magnetic Resonance Imaging for Measuring Cross-Sectional Area

Multiple sclerosis (MS) refers to the lesions that accumulate in the brain and spinal cord. Magnetic resonance imaging (MRI) is the most sensitive and versatile modality used to show changes in the tissues over time. There has been significant interest in evaluating the relationship between the brain atrophy and disease progression rather than the spinal cord atrophy. The cervical spinal cord h...

Cervical and ocular vestibular evoked myogenic potentials in multiple sclerosis participants

Background: Multiple sclerosis (MS) is a chronic neurological disease that affects brain and spinal cord. The infratentorial region contains the cerebellum and brainstem. Vestibular evoked myogenic potentials (VEMPs) are short-latency myogenic responses. Cervical vestibular evoked myogenic potential (cVEMP) is a manifestation of vestibulocolic reflex and ocular vestibular evoked myogenic pote...
